Center Township, Indiana is here to help if you are in a financial emergency situation. We can provide assistance with rent and utilities if you have an eviction or disconnect notice.

Our team may also refer you to additional resources, training programs and local charities to help you get back on track.

Eligibility for assistance is determined on a case by case basis but everyone must meet the following criteria:

  1. Be a Center Township resident.
  2. Provide completed forms and documents required for the application (see list by category).
  3. Sign application form. All adults in the household must sign the form.
  4. Discuss completed application in person with a Case Coordinator.​

1. Rental Assistance

  • Social security cards for all members living in your household.
  • ID/driver’s license for everyone that is of age in household.
  • Copy of most current taxes
  • Proof of income at least 1 month (all members) in household
  • Receipts for rent paid in the last 2 months
  • Copy of bank statements/checking & savings
  • Divorce/or separation papers
  • Custody of children court papers
  • Attached Landlord Letter accepting voucher
  • Copy of full lease
  • Doctor’s excuse (if that is the reason you are not working)
  • Job search sheet completed; this is the blue sheet in the application packet.
  • Monthly allowance record sheet for food stamps
  • Monthly allowance records on any other government agency help (ICAPP)
  • Anyone receiving social security. We need your record sheet of amount you receive

2. Utilizes Assistance

  • Social security cards for all members living in your household.

  • Monthly allowance records on any other gov agency help (ICAPP)
  • Anyone receiving social security. We need your record sheet of amount you receive.

3. Medical Services

The township will, in cases of necessity, promptly provide medical assistance for qualifying Township Assistance applicants who are not provided for in public institutions; have coverage under a private insurance policy; or receiving governmentally subsidized medical benefits, such as: Medicaid or Medicare. The township shall only pay the cost of the following medical services for the eligible and qualifying poor of the township:

4. Funeral Assistance

  • Proof of residence
  • Death Certificate
  • Invoice From the funeral home
  • ID & Social Security Card
  • Previous Income
